About This Agreement

A Business Associate Agreement (BAA) is the contract required by HIPAA between a covered entity (or another business associate) and a service provider that creates, receives, maintains, or transmits Protected Health Information (PHI) on its behalf.

UM-AI executes a BAA with every customer organization whose use of the service involves PHI, before that organization sends PHI to the platform. The BAA covers the safeguards, permitted uses, breach-notification obligations, and termination duties required by 45 CFR §§ 164.502(e) and 164.504(e), and flows down equivalent obligations to our sub-processors.

To request the executed BAA template, or to coordinate signature for your organization, email privacy@umgpt.ai.

Important Notice

This template is provided for reference and informational purposes. An executed BAA requires countersignature by both parties. To request a countersigned BAA for your organization, contact privacy@umgpt.ai. This BAA is an addendum to the Terms of Service or a separately executed Master Service Agreement (MSA).
